The Income Withholding Order form can be downloaded from the website of the Office Of Child Support Enforcement.

• This IWO form is the standard format prescribed by the Secretary of Health and Human Services in accordance with USC 42 §666(b)(6)(A)(ii). The Office-of-Management-and-Budget-approved IWO must be issued to employers or other income payers to collect child support.
