The Judicial Council has the statutory authority to prescribe many of the forms used in legal proceedings. Forms will be downloaded into a new window. Choose from forms in PDF, WordPerfect or Rich Text Format (RTF) which will open with Microsoft Word.

Probate, guardianship and conservatorship forms are contained in the Kansas Judicial Council Probate Forms 2d publication. Kansas lawyers registered as active under S. Ct. Rule 208 may access these forms by clicking here.

Please be advised that Kansas Court Clerks and Court staff MAY NOT provide legal advice, recommend any specific course of legal action, assist in completion of any forms or advise on how a particular term or definition applies to a specific situation, or interpret statutes or rules or advise whether a particular statute or rule applies in a specific situation. They may not provide information kept confidential by statute or court rule nor may they recommend or advise concerning rules of evidence, witnesses, objections, or rulings. See Administrative Order No. 232.
