Pursuant to 2005 SB 258 and 2006 SB 550, a number of forms were removed from volumes 4 and 4A of the Kansas Statutes Annotated. Those forms, appearing below, are to be set forth by the Judicial Council pursuant to K.S.A. 59-2143.
