The following Child in Need of Care forms were prepared by former Judge Kathryn Carter of Jamestown and reviewed by the Judicial Council Juvenile Offender / Child in Need of Care Advisory Committee. These forms are the final version and are available in PDF, WordPerfect and Rich Text Format (RTF).

The Committee is aware that, despite its best efforts, no set of forms is perfect for every situation and wishes to emphasize that forms are not a substitute for thinking by court and counsel.
